MetroHealth Medical Center Faculty Orientation Documents
All students and faculty must complete the NEONI Student Orientation prior to doing clinical rotations. Schools of nursing should submit a letter attesting to the successful completion of the NEONI student orientation prior to the beginning of clinical rotations which will be held in the MetroHealth System.
All clinical instructors will need to complete online education programs yearly.The courses must be completed for that year prior to beginning any clinical rotation. Clinical instructors will not be allowed to begin any clinical rotation until they have completed the online education.
A letter must be presented to the Manager of Nursing Professional Practice which attests to the presence of valid clear background check, and vaccines that are up to date for all nursing students, (in accordance with MetroHealth System Policies). It is not necessary that copies of background checks or vaccines be submitted, but records should be on file at the schools of nursing, and available upon request. The Attestation letter must be received no later than two weeks prior to the beginning of clinical rotations. (Please see section on Hospital Policies – Attestation letter). Any questions regarding background checks should be submitted to the Manager of Nursing Professional Practice no later than 3 weeks prior to the first day of the clinical rotation. Access to a clinical area will be denied until the Attestation Letter has been received.
All clinical instructors should make sure that the schools of nursing have submitted the rosters for their clinical groups along with the Acknowledgement of policy forms. These forms should be received no later than three weeks before the first day of clinical rotations. Any delays in the submission of BOTH of these forms will cause delays in obtaining Epic passwords for the students and will delay obtaining Identification badges. (See Attachment B)
All instructors must have EPIC access prior to beginning clinical rotations.
- Instructors who have not logged into the Epic site within the past 90 days must request a new password. This can be accomplished by filling out and submitting
The Request for Epic Security for Nursing Faculty form. (See Attachment E)
- Instructors who are beginning their first rotations at MetroHealth, or who have not taken the class on the Epic system will be required to take the class prior to the beginning of any clinical rotation. Any new instructors who do not take the Epic class will NOT be allowed to begin clinicals.
- When Epic authorization has been received, it will be made available to the instructor for pick up in the Nursing Business Office (4th floor Hamann Building). Please contact Cheryl Malleo, Manager of Nursing Professional Practice, at extension 83291 to arrange a date for pickup.
- It is the responsibility of ALL instructors to contact the Nurse Mangers of the floors on which they will be doing clinical rotations with students. (See Attachment A)
- A one day orientation (Labor and Delivery requires two days) is required for all nursing instructors prior to beginning clinical rotations on any unit or floor. The instructor should meet with the Nurse Manger at least yearly to discuss any unit updates or changes. Arrangements must be made in advance between the instructor and the nurse Manager.
- The instructor will provide the Nurse Manger with the course objectives, focus of the experience, the skills the students currently have, and the skill which they need to acquire.
- Education Partners requesting clinical placements should refer to these unit descriptions. Please search the database by your desired unit for availability prior to submitting a new or change placement request in StudentMAX™.
Pyxis access is available for instructors only. Instructors who have been away from MetroHealth for more than six months will need to submit the Pyxis Utilization Release form (See Attachment D) and submit proof of a background check that is less than one-year-old. The Pyxis form and proof of a background check should be faxed to the attention of Cheryl Malleo at 216.778.3726 at least 2 weeks prior to the beginning of the clinical experience. The initial password will be “pyxis,” when logging on for the first time. You will then select a new password and scan your finger. For instructors who have not logged onto the pyxis system in the past 90 days, A Pyxis Utilization Release form must be filled out with the appropriate box checked at the top of the form. Write in at the top that a background check is already on file.
All students must complete the challenge questions prior to utilizing the MAR for medication administration. The challenge questions need to be completed in the Nursing Business Office. Please call the secretary in the Nursing Business Office (x 85436) to make sure that the computer is available for use.
If students will be required to do Blood Glucose monitoring during their clinical rotation, the instructor must contact Darcy Rohr prior to the beginning of clinical rotations. (See attachments F1 & F2)
- The instructor will be competency tested on BGM monitoring and will be responsible for competency testing the students.
- The instructor should contact Darcy Rohr to schedule BGM competency testing at 216.778.5403 or drohr@metrohealth.org.
- After students have received their MH Identification Badges, MH Police will fax the form containing their Identification numbers to Darcy Rohr.
- Once the Temporary ID form has been received by Darcy Rohr, the instructor can begin the competency testing of the students.
Acknowledgement of policy forms: All acknowledgment forms must be submitted to Cheryl Malleo no later than three weeks prior to the beginning of clinical rotations. Once forms for the clinical group have been received, Epic access will be released to instructors. (See Attachment C) (See Attachment J)
Clinical Rosters and Acknowledgment of policy forms must be submitted to the Manager of Nursing Professional Practice, in order for Identification badges to be issued. Arrangements must be made by contacting the MH police at 216.778.3006 for students to have Photo ID’s taken. All persons are required to have proof of identity in order to obtain a MetroHealth ID. Valid forms of identification are:
- Valid Drivers License
- State Issued Identification Card
- U.S. Passport
- U.S. Visa
The MetroHealth Identification Badge must be worn at all times during all clinical experiences at any of The MetroHealth System facilities. Students whom are employed as Clinical Assistants at MetroHealth will be issued and additional student badge for use when they are here for clinicals.
At the end of the clinical experience, all ID badges must be collected and turned into the MH police office. Students will receive new ID badges at the beginning of each clinical rotation.
Hospital Policies
Instructor Online Education:
Beginning the Fall Semester of 2012, all instructors will be required to complete yearly competencies using our Learning Management System (LMS). Instructors are required to successfully complete the required courses prior to the beginning of clinicals. The Medication / Math test must be passed with a score of 90% or greater. Anyone who does not achieve a score of 90% or greater will not be allowed to oversee clinicals for that semester. Prior to returning to MetroHealth for subsequent semesters, the Medication / Math test will need to be repeated and success fully passed prior to the next rotation that they wish to supervise. MetroHealth employees, who serve as instructors, will also need to complete the sections that are required for all instructors prior to the beginning of their first clinical rotation. They will also have to pass the Medication / Math test yearly. Clinical instructors will not be allowed to begin any clinical rotation until they have completed the online education.
The LMS can be accessed from any computer.
Attestation Letter
A letter must be received from each school of nursing with clinical rotations at MetroHealth Medical Center. The letter must attest to and contain the following information:
- Valid Background checks for students and instructors in attendance at Metro. Background checks must be clear. Background checks that are not clear need to be discussed with the Manager of Nursing Professional Practice at least two weeks before the beginning of the clinical experience.
- All vaccinations must be listed as up to date, inclusive of the influenza vaccine.
- Passage of the NEONI orientation test by all instructors and students with passage of 90% or higher. Please list date of the yearly test.
- BLS for healthcare providers currently active.
- Presence of liability insurance.
- Affirmation that all students are in good standing.
Parking:
Students can purchase a parking pass at the Parking Office during the hours of 8 a.m. and 3:30 p.m. Please contact the parking office at 216.778.4600 to make arrangements. (See Attachment G)
Alaris IV Pumps:
MetroHealth utilizes the Alaris IV Pump system. All nursing instructors should be familiar with the pump prior to instructing students on its use. We have attached the workbook for the pump, for use by instructors and students. Once you have completed the workbook, please contact Cheryl Malleo for further questions at 216.778.3291. (See Attachment I)
Conference Rooms:
Conference rooms are available on the clinical units and are scheduled with the Nurse Manager. Additional conference rooms are available in the Rammelkamp Center for Education and Research and may be scheduled through HR – Organizational Development. Send an email to smcgraw@metrohealth.org with the dates, times and group size to request a room. Availability will be checked and you will be sent a confirming e-mail containing your room assignment. The keys are obtained from the MetroHealth Police Department.
Medication Administration:
All medication administration by nursing students must be strictly supervised by the nursing instructor. All medications should be administered only if the nursing instructor is in attendance. Students should be made aware of how to access all policies and reference materials as they pertain to medication administration. In case of a medication error the student should submit any hospital required paperwork with the assistance of the nursing instructor. The nursing instructor must also submit a note to the Manager of Professional Nursing Practice detailing the incident and involved parties.
Student Assignments:
A student assignment form must be filled out daily by nursing at least 15 minutes prior to the beginning of the student experience. Student assignments should be discussed with the charge nurse prior to the student’s arrival on the unit. Please utilize the assignment sheet which is specific for nursing students. (See Attachment H). A copy of the student assignment must be attached to the staff assignment for that day and is to remain on the unit for reference.
The Brittingham Library:
The Brittingham Library is available for student use, however, students may not check out materials. Coin operated copy machines are available; the student must bring their own change. The library is located on the second floor of the Rammelkamp building.
MetroHealth Policies/Portable Devices:
While at MetroHealth, the students are expected to follow all MetroHealth rules, regulations, policies and procedures. Instructors are charged with the responsibility for providing an orientation for students, with emphasis on conformance with MetroHealth policies regarding confidentiality, patient rights, safety, emergency procedures, safety goals and Portable Computing Devices. In accordance with the MetroHealth Policies, any infractions will be dealt with individually.
Students may not use portable computing devices unless these devices meet the current MHS Portable Device standards.
During the Clinical Experience
- If an instructor is ill or out of the hospital, students may not be on the unit. Instructors are not permitted to utilize MetroHealth nursing personnel to substitute in their absence. Instructors should notify the nurse manager if students will not be present for the scheduled clinical experience.
- Promptly report unusual occurrences, incidents or errors on the clinical unit to the charge nurse and/or nurse manager.
Emergency Medical Care
Emergency medical care is available for instructors and students, at their own expense, for injury occurring to them while they are participating in the scheduled clinical experience on the hospital premises. The injury should be reported to the nurse in charge and the student/instructor will be sent to the Medical Clinic or Emergency Department as appropriate. If the Medical Clinic is closed, call the nursing supervisor.
Health Criteria
The records validating these requirements are maintained by the affiliating school and made available to The MetroHealth System upon request.
Proof of immunity as follows:
- Measles: An individual is considered to be immune to measles if the individual has:
- a positive antibody titer, or
- received two doses of live measles vaccine or two doses of measles, mumps, rubella (MMR) vaccine.
- Rubella: an individual is considered immune to rubella if the individual has:
- a positive antibody titer, or
- received a single dose of rubella vaccine or
- Varicella: an individual is considered immune to varicella if the individual has:
- a positive antibody titer, or
- received two doses of varicella vaccine
- An initial two step mantoux test and if negative, an annual mantoux TB test thereafter.
- If TB test is positive the student was referred for appropriate evaluation to ensure they do not have active TB.
Influenza Vaccine
The influenza vaccine is available to students/instructors from October through January each year through the Employee Health Clinic. All students are required to have obtained the vaccine, or they will be required to wear a face mask when on any patient care area. If vaccinations are obtained at a facility outside of MetroHealth, students will be required to provide proof of receiving the vaccine. They will then be provided with a flu fighter pin.
Requests for Clinical Units
All requests, for group clinical experiences (including observational experiences), are submitted through the NEONI online clinical placement tool. The identified school coordinator requests clinical placements.
To access the NEONI Online clinical Placement site:
- Go to: www.neoniplacement.org
- You will be able to view the clinical placements that are currently scheduled.
- Utilization of clinical areas will be determined by the director of nursing, and nurse manager of each clinical area.
- Requests are processed and confirmed through the NEONI online clinical placement tool.
- A maximum faculty/student ratio of 1:8, except as noted below, is permitted. The ratio is subject to the approval of the nurse manager and may be lowered.
- Labor & Delivery, ratio 1:5.
- One-two students may be allowed in specialty units and/or ambulatory clinics for observational experiences.
Nursing Instructor Co-signature
It is requires that all of the notes that the students place in the electronic medical record be co-signed by the nursing instructor daily. This can be accomplished by a single note at the end of each clinical day. Please find directions to accomplish this below.
- Go to the Notes section of the patient’s record.
- Select New Note.
- Type the dot phrase .nursinginstructor (no space between these words)
- This smart phrase will appear: "I have reviewed and agree with ***'s (Student Nurse) documentation for *** shift."
- Replace the *** with the student’s name and the period of time for the clinical rotation. (example: I have reviewed and agree with Jill Smith’s documentation during the 0700-1100 period of care.)
