STEP 3:  Execute the Service Agreement

This Online Student Nurse Clinical Placement Service Agreement (“Agreement”) is made by and between The Center for Health Affairs - Northeast Ohio Nursing Initiative (NEONI) with an address of 1226 Huron Road East, Cleveland, Ohio 44115 and a school of nursing or a clinical agency (“User”). 

This agreement allows hospitals and schools to utilize the StudentMAX™ software to arrange clinical site placements. This document is separate from the traditional school/facility contract.
